Description

Arising from the superior mesenteric artery, the middle colic artery courses superiorly and anteriorly within the transverse mesocolon, then divides into right and left branches that track along the mesenteric border of the transverse colon. From a posterior perspective, the transverse colon sits superior to the small intestinal loops, while the ascending and descending colon frame the abdomen laterally, with the sigmoid colon curving inferiorly toward the rectum. Along the colonic margin, the marginal artery (of Drummond) forms an arterial arcade that links the middle colic territory with the right colic and left colic arteries. Accompanying veins, rendered in blue, parallel the arterial pattern and drain toward the superior mesenteric vein and portal circulation. Posterior visualization of the middle colic pedicle matters when you are thinking like a colorectal surgeon. Mobilization of the transverse colon, takedown of the hepatic or splenic flexure, and lymphadenectomy for transverse colon cancer all hinge on where the middle colic originates and where its bifurcation lies within the mesocolon. Injury or ligation too close to the marginal artery can leave a watershed segment at the splenic flexure vulnerable to ischemia, an issue that becomes obvious when you trace the anastomotic continuity of the marginal artery. This is a common point of failure.
