Description
Arising from the superior mesenteric artery, the middle colic artery courses anteriorly and superiorly into the transverse mesocolon, where it divides into right and left branches that run along the mesenteric border of the transverse colon. The transverse colon occupies the upper portion of the field, with the ascending colon positioned on the patient’s right and the descending colon on the left, continuing inferiorly toward the sigmoid colon and rectum. Blue venous tributaries accompany the arterial tree within the mesentery, while the greater omentum, rendered as yellow adipose tissue, drapes anterior to the colonic loops. Pelvic bones anchor the inferior margin. For colonic perfusion, the middle colic artery is the key conduit to the midgut-derived transverse colon and a major contributor to the marginal artery of Drummond, where it anastomoses with right colic and left colic territories near the hepatic and splenic flexures. This is surgical anatomy. During transverse colectomy, extended right hemicolectomy, or complete mesocolic excision, surgeons ligate the middle colic trunk or its branches near their origin, balancing oncologic lymphadenectomy against the risk of ischemia to the remaining transverse colon. The same vascular crossroads becomes unforgiving in low-flow states, with watershed vulnerability around the splenic flexure when collateral pathways are limited. Ideal for teaching mesenteric vascular territories in gross anatomy, colorectal surgery, and GI radiology curricula, this anterior perspective also supports textbook figures and consent materials that explain central vascular ligation and anastomotic risk after colonic resection.
