Description
Sweeping along the medial aspect of the cerebral hemisphere, the anterior cerebral artery (ACA) is presented in lateral profile as it courses anterosuperiorly from the internal carotid system toward the longitudinal fissure, then tracks posteriorly over the superior surface of the corpus callosum. Branching arterioles extend toward the medial frontal and parietal cortices, while the vessel’s relationship to the cingulate gyrus and pericallosal region reads clearly in this projection. Superficial cortical gyri and sulci provide orientation, with the brainstem and spinal cord continuing inferiorly into the cervical region, and yellow-coded cranial and spinal nerves contrasting against red arterial and blue venous pathways. Spatially, the ACA sits medial to the lateral convexity cortex and superior to the optic chiasm region, consistent with its deep midline trajectory. For teaching cerebrovascular territories, this is the view you want. The ACA’s pericallosal course over the corpus callosum explains why infarcts often present with contralateral leg-predominant weakness and sensory loss from involvement of the medial precentral and postcentral gyri, and why abulia or akinetic mutism can appear when medial frontal and cingulate areas are ischemic. A short segment near the anterior communicating artery is a common site for saccular aneurysms, and lateral orientation helps learners connect aneurysm location with patterns of subarachnoid hemorrhage distribution and nearby perforator risk to basal forebrain structures. Small vessel caliber changes matter here.
