Description
Arising from the internal carotid artery, the middle cerebral artery (MCA) courses laterally to enter the lateral sulcus (Sylvian fissure), where its M1 segment gives off lenticulostriate branches toward the basal ganglia and internal capsule before dividing into M2 insular branches. From the side, the vessel fans superiorly and posteriorly over the cerebral convexity as M3 opercular and M4 cortical branches, tracking along the sulci to supply the lateral frontal, parietal, and superior temporal lobes. Superficial cerebral veins and dural venous channels appear in parallel as blue drainage pathways, while yellow cranial nerves emerge from the brainstem inferiorly, with the spinal cord continuing caudally. Clinically, this lateral perspective maps directly onto the common distribution of large-vessel ischemic stroke, where proximal MCA occlusion threatens the lateral sensorimotor cortex and language cortex in the dominant hemisphere. M1 lenticulostriate rupture is a classic source of hypertensive intracerebral hemorrhage, and their deep, end-artery pattern explains the small but disabling capsular infarcts seen with lacunar disease. Endovascular thrombectomy planning also depends on understanding the M1 to M2 bifurcation geometry and the relationship of opercular branches to the Sylvian fissure. A frequent site of injury.
