Seen in lateral section through a partial male skull, the middle cerebral artery (MCA) arises from the internal carotid artery and courses laterally to enter the lateral sulcus (Sylvian fissure), where its M1 segment gives perforating lenticulostriate arteries medially toward the basal ganglia and internal capsule. Distal branches fan superiorly and posteriorly over the insula and opercular cortices, then distribute across the lateral convexity of the frontal, parietal, and temporal lobes. Superficial cortical veins and dural venous channels are suggested in blue, positioned more superficially than the arterial tree, with calvarial bone margins framing the intracranial compartment. Spatial relationships stay intuitive. Cortex lateral, deep perforators medial. MCA anatomy is where cerebrovascular localization stops being abstract and becomes practical: occlusion proximal to the bifurcation can compromise the lenticulostriate territory and produce dense contralateral hemiparesis from internal capsule infarction, while more distal branch infarcts track with aphasia in the dominant hemisphere or hemispatial neglect in the nondominant hemisphere. The artery’s deep course within the Sylvian fissure also matters for neurosurgical corridors, since splitting the fissure during aneurysm clipping exposes the MCA trunk and bifurcation while demanding respect for fragile perforators. Small structures, big consequences.
