Description
Running along the medial surface of the cerebral hemisphere, the anterior cerebral artery courses anterosuperiorly from the internal carotid artery and then sweeps posteriorly over the corpus callosum as the pericallosal artery, with the callosomarginal branch turning superiorly in the cingulate sulcus. Distal pericallosal and callosomarginal branches arborize over the superior frontal gyrus and paracentral lobule, their cortical twigs extending posteriorly toward the medial parietal region. Nearby venous channels track back in blue, and yellow neural elements frame the brainstem and proximal spinal cord, giving vascular landmarks clear relationships to the midline neuraxis. Orientation stays lateral to oblique, but the distribution is unmistakably ACA territory. Clinically, this is the view you want when teaching why ACA stroke patterns look the way they do. Occlusion or embolus affecting the pericallosal or callosomarginal branches preferentially injures the medial frontal cortex and paracentral lobule, producing contralateral leg-predominant weakness and sensory loss, and may add abulia from cingulate involvement. A small anterior communicating artery aneurysm, just proximal to the A2 segment, can compress the optic chiasm and is a common target in microsurgical clipping and endovascular planning. Small vessels matter here. Even subtle caliber changes can cue vasospasm after subarachnoid hemorrhage. Neuroanatomy and neurovascular courses often pair this plate with sagittal brain sections, circle of Willis diagrams, and infarct maps in medical school and neurology clerkships. It also reads well in stroke service teaching files, neuroradiology reports that reference ACA segments, and neurosurgical lectures on interhemispheric approaches and pericallosal aneurysms.
