Description
Seen from a lateral perspective, the descending branch of the lateral circumflex femoral artery courses inferiorly along the anterolateral thigh, arising near the profunda femoris and tracking toward the lateral aspect of the knee. Proximally it lies deep to the tensor fasciae latae and sartorius, then runs in the interval between rectus femoris medially and vastus lateralis laterally, where it gives muscular branches and perforators toward the vastus lateralis. Distally, the vessel approaches the lateral femoral condyle and participates in the periarticular anastomotic network around the knee, with the femur, patella, tibia, and fibula providing firm osseous landmarks beneath the overlying quadriceps and lateral hamstring contour. Orientation is clear. This artery matters because it is the dominant pedicle for the anterolateral thigh (ALT) flap, and its perforators through the vastus lateralis dictate flap design, intramuscular dissection time, and the risk profile for donor-site morbidity. Orthopedic and sports surgeons also care about its distal contribution to the lateral genicular anastomoses when working around the distal femur and patellar tendon, where aggressive lateral retinacular release, hardware placement, or traumatic dissection can compromise regional perfusion. For vascular teaching, it is a clean example of how the profunda femoris system supplies the anterior compartment without following the main femoral artery into the adductor canal.
