Description
Arising from the lateral circumflex femoral artery off the profunda femoris, the descending branch tracks inferiorly along the anterolateral thigh, running deep to the rectus femoris and between vastus lateralis and vastus intermedius toward the lateral aspect of the knee. An anterior perspective also places the femoral artery and vein in the femoral triangle, with the great saphenous vein coursing superficially on the medial side as it approaches the saphenofemoral junction. Bony landmarks such as the acetabulum and proximal femur superiorly, the patella anterior to the distal femur, and the tibia and fibula distal to the knee provide clear orientation in anatomical position. Surgeons care about this vessel because the descending branch supplies the vastus lateralis territory and forms a reliable vascular axis for the anterolateral thigh (ALT) flap, a workhorse in head and neck and extremity reconstruction. Its distal contribution to the lateral genicular anastomosis also matters when planning approaches around the knee, and iatrogenic injury can occur during lateral thigh incisions or harvest if the perforators are not mapped.
