Description
Arising in the proximal leg, the circumflex fibular artery courses anterior to the fibular neck and then loops laterally around the head of the fibula, hugging the periosteal contour before joining the periarticular vascular network of the knee. Proximal to it lie the distal femur and the patella on the anterior aspect, while the tibia sits medial to the fibula and forms the main weightbearing column of the leg. The vessel’s arc sits deep to the superficial fascia and runs near the attachments around the fibular head, a region shared by the lateral collateral ligament and the biceps femoris tendon. Small arteriolar branches extend distally toward the anterior compartment and proximally toward the inferior margin of the knee joint capsule. That looping course matters because the fibular head is a busy surgical neighborhood. During lateral approaches to the knee, proximal tibiofibular joint procedures, or fixation of fibular head and neck fractures, this vessel can be injured, creating a brisk hematoma that obscures the plane and complicates identification of adjacent structures such as the common fibular (peroneal) nerve as it winds around the fibular neck. It also helps explain periarticular collateral circulation when popliteal flow is compromised, and why bleeding may persist despite proximal tourniquet control if collateral channels remain patent. A small artery, but an unforgiving one.
