Circumflex Fibular Artery, Anterior View

Description

Running anterolaterally from the popliteal region, the circumflex fibular artery curves around the proximal fibula near the fibular head and neck, tracking close to the lateral aspect of the knee joint line. An anterior perspective brings the distal femur and proximal tibia into alignment at the tibiofemoral articulation, with the fibula positioned lateral to the tibia as a clear bony landmark for the vessel’s course. Adjacent arterial branches around the knee, consistent with the genicular network from the popliteal artery, accompany paired venae comitantes shown in blue. Relationships to the lateral collateral ligament and the biceps femoris tendon at the fibular head are implied by the vessel’s proximity to the posterolateral corner. Attention to the circumflex fibular artery matters when you are teaching or operating around the fibular head, where vascular structures sit near common sites of iatrogenic injury. Proximal fibular fracture fixation, lateral meniscal surgery, and posterolateral corner reconstructions all work in a corridor where small arterial branches can bleed briskly and obscure the field. It is also a useful reminder that knee perfusion is redundant, but not uniform, and an anterior map of periarticular vessels supports discussion of anastomoses during trauma or after popliteal artery compromise. Bleeding here is memorable.
