Description
Seen from a posterior perspective, the male pelvis and proximal femur frame the pectineus as a short, flat muscle bridging the superior pubic ramus to the proximal femur. Its origin lies on the pecten pubis and adjacent pubic crest, positioned anteromedial to the hip joint, while its insertion runs to the pectineal line of the femur just inferior to the lesser trochanter. From this angle, the pectineus sits medial to the iliopsoas tendon and anterior to the adductor longus and adductor brevis, with the femoral head seated in the acetabulum lateral to its course. Deep and compact. Pectineus matters because it occupies the borderland between the anterior and medial compartments of the thigh, so it often becomes the “missing piece” when teaching why hip flexion and adduction can be coupled in gait and in resisted straight leg raise. Clinically, its deep location near the superior pubic ramus and hip capsule makes it a common but under-recognized pain generator in athletic groin strain patterns, and a key structure to differentiate from adductor longus pathology on exam and imaging. For surgeons, its proximity to the femoral neurovascular bundle and its variable innervation (femoral nerve, obturator nerve, or both) are practical considerations during approaches to the femoral triangle and medial hip.
