Description
Positioned in the superior medial thigh, the pectineus is presented from an anterior perspective in a male, lying just inferior to the pubic crest and superior ramus of the pubis and running inferolaterally to the pectineal line of the femur. Its broad proximal attachment sits medial to the iliopsoas tendon as it passes toward the lesser trochanter, while the distal fibers overlap the proximal adductor longus and approach the adductor brevis deeper and more posteriorly. Laterally, the femoral shaft provides the bony reference for the muscle’s insertion; medially, the muscle forms part of the floor of the femoral triangle. Small muscle. Big landmark. Teaching the pectineus well matters because it sits at the junction between the anterior and medial compartments, and that boundary is where students and trainees often misassign hip flexion versus adduction mechanics. Clinically, anterior groin pain can arise from pectineus strain or enthesopathy at the pubis or femur, and the muscle’s relationship to the femoral nerve, femoral artery, and femoral vein makes it relevant when interpreting femoral triangle tenderness, post-catheterization hematoma, or planning an anterior approach to the hip. Its variable innervation, commonly femoral nerve with frequent contribution from the obturator nerve, also explains mixed sensory and motor findings in proximal nerve lesions.
