Description
Positioned at the superomedial thigh, the pectineus forms a flat, quadrilateral muscular sheet bridging the anterior hip region to the proximal femur. From an anterior perspective you would expect it to lie just inferior to the inguinal ligament and medial to iliopsoas, with its distal fibers approaching the pectineal line of the femur while the adductor longus sits immediately medial and the adductor brevis deeper and posterior. Laterally, the femoral triangle boundary is implied, with pectineus contributing a muscular floor beneath the femoral vessels and nerve. Clear borders matter. Pectineus sits at the intersection of hip flexion and adduction, which makes it a frequent point of confusion when teaching adductor compartment anatomy and when interpreting groin pain patterns. Its close relationship to the femoral neurovascular bundle and the pectineal (Cooper’s) ligament gives this view practical relevance for femoral hernia repair, vascular access in the femoral triangle, and regional anesthesia techniques where muscle planes guide needle placement. Surgeons also use the pectineal line and adjacent muscular attachments as landmarks during anterior approaches to the hip and during exposure of the superior pubic ramus.
