Description
Positioned in the superomedial anterior thigh, the pectineus forms a flat, quadrangular muscle that runs from the superior pubic ramus (pecten pubis) inferolaterally to the pectineal line on the proximal femur, immediately inferior to the lesser trochanter. Its belly lies anterior to adductor brevis and adductor longus, while iliopsoas sits more laterally and deep, passing toward the lesser trochanter. Medially, the pectineus helps define the floor of the femoral triangle, with the femoral vessels lying anterolateral to the adductor plane as they transition into the adductor canal. Bone landmarks anchor the view. Clinically, this anterior exposure matters because pectineus is a frequent site of proximal groin pain and partial tears in kicking and sprinting athletes, and its location can confuse diagnoses when symptoms overlap with iliopsoas tendinopathy or adductor longus enthesopathy at the pubis. Its dual innervation is a teaching point: most commonly femoral nerve (L2-L3) with a variable obturator contribution, so selective nerve blocks or neuropathies can spare or weaken hip flexion and adduction in patterns that do not match the adductor compartment cleanly. Surgeons also use the pectineal line and adjacent femoral neurovascular bundle as orientation during anterior approaches to the hip and when evaluating femoral hernia relationships.
