Description
Oriented from a posterior perspective, the forearm is shown in extension with the radius lateral and the ulna medial, providing a clear corridor for the posterior interosseous veins along the dorsal aspect of the interosseous membrane. Slender venules coalesce into a longitudinal plexus deep to the extensor compartment, tracking between the proximal carpal row inferiorly and the cubital region superiorly. Bony landmarks of the humerus, elbow joint, radius, ulna, and the metacarpals and phalanges anchor the course of the deep veins in relation to the skeletal framework. Deep venous drainage of the posterior compartment matters when you are teaching the difference between superficial dorsal venous networks and the venae comitantes that accompany the posterior interosseous artery in the deep plane. This view also supports surgical and procedural anatomy around the proximal dorsal forearm, where posterior interosseous neurovascular structures are encountered during approaches to the radial head, proximal radius, and dorsal wrist, and where postoperative hematoma or swelling can compromise local venous return. Deep and superficial systems connect, but they behave differently.
