Description
Beneath the posterior skin of an adult male upper back, the superficial trapezius spans from the external occipital protuberance and nuchal ligament down to the thoracic spinous processes, its fibers running laterally toward the clavicle, acromion, and spine of the scapula. Deep to the trapezius, the rhomboid major forms a broad quadrilateral sheet extending from the spinous processes of T2 to T5 to the medial border of the scapula, inferior to the smaller rhomboid minor. Medially, the thoracic vertebrae and midline fascia anchor these muscles, while laterally the scapula sits over the posterior thoracic wall and defines the plane of scapular retraction. Posterior layering matters here because the rhomboid major cannot be appreciated clinically without accounting for the trapezius overlying it, a relationship that guides both palpation and dissection. Scapular winging from long thoracic nerve palsy (serratus anterior weakness) shifts the medial border of the scapula posteriorly and changes the resting length of the rhomboids and trapezius, altering retraction mechanics and often producing interscapular pain. Clear muscular boundaries also support discussion of dorsal scapular nerve entrapment along the medial scapular border, a less common but specific cause of focal medial scapular aching and weakness in retraction.
