Description
Posterior upper thorax and shoulder girdle are rendered in a male figure, with the trapezius dominating the superficial layer from the occiput and nuchal ligament to the lateral clavicle, acromion, and scapular spine. Deep to its middle fibers, the rhomboid major is positioned inferomedial to the scapular spine, coursing from the spinous processes of T2 to T5 vertebrae toward the medial (vertebral) border of the scapula. The rhomboid major lies inferior to rhomboid minor and medial to the scapular body, while the deltoid caps the lateral shoulder and frames the scapular attachments in relation to the proximal humerus. Bony landmarks such as the clavicle and scapular margin anchor the muscle topography. Rhomboid major anatomy matters most when you are teaching scapulothoracic mechanics or sorting out posterior shoulder pain. Retraction and downward rotation of the scapula depend on coordinated firing of trapezius, rhomboids, and serratus anterior, and imbalance often appears clinically as altered scapular positioning during abduction with secondary subacromial symptoms. Dorsal scapular nerve palsy or traction injury can weaken the rhomboids, producing subtle lateral drift of the medial border and fatigue with sustained posture. Small muscle, big consequences.
