Description
Positioned in the anterior neck but presented from a posterior perspective, the right and left thyroid lobes sit on either side of the trachea, joined across the midline by the isthmus and invested by a thin capsule. Superior thyroid arteries descend toward the superior poles, while inferior thyroid arteries ascend from the thyrocervical trunks to reach the posteroinferior aspects of each lobe. Posterior venous channels accompany the arterial pattern, with the trachea lying anterior to the gland and the esophagus immediately posterior and slightly left of midline. Orientation is clear. Posterior arterial anatomy of the thyroid matters because this is where operative planes develop during thyroidectomy and where vascular control is obtained close to the capsule to protect adjacent nerves. The inferior thyroid artery commonly crosses near the recurrent laryngeal nerve at the level of the cricothyroid junction and tracheoesophageal groove, so depicting its ascent to the lower pole helps explain why nerve monitoring and careful ligation strategy reduce postoperative vocal fold paresis.
