Description
Arising from the common carotid circulation, the superior thyroid arteries descend to the superior poles of the right and left thyroid lobes, while the inferior thyroid arteries sweep medially from the thyrocervical trunks toward the posteromedial gland, branching across the thyroid capsule toward the isthmus. A posterior perspective brings the parathyroid glands into clear view, typically two superior and two inferior nodules resting on the posterior surface near the junction of the middle and lower thirds of each lobe. Medial to the gland, the tracheal rings form the midline airway framework, with the recurrent laryngeal nerves ascending in the tracheoesophageal grooves toward the larynx, closely related to terminal branches of the inferior thyroid artery. Small arterioles course within the capsular plane. Tight anatomy. This relationship matters in thyroidectomy, where ligation strategy and dissection plane directly affect voice and calcium outcomes. Many surgeons divide the superior thyroid vessels close to the thyroid to protect the external branch of the superior laryngeal nerve, then identify the recurrent laryngeal nerve near Berry’s ligament and the inferior thyroid artery branches, a frequent crossing point and a common site of thermal or traction injury. Parathyroid viability also hinges on preserving the delicate posterior vascular arcade, since devascularization can precipitate postoperative hypocalcemia even when the glands are left in situ.
