Description
Positioned beneath the skin of the male posterior neck and upper back, the trapezius spans from the external occipital protuberance and superior nuchal line inferiorly along the midline to the nuchal ligament and spinous processes of the cervical and thoracic vertebrae. Laterally, its fibers converge toward the shoulder girdle, inserting along the lateral third of the clavicle, the acromion, and the spine of the scapula, forming clear superior (descending), middle (transverse), and inferior (ascending) portions. The right and left muscles mirror each other across the vertebral column, framing the cervical spine superiorly and the upper thoracic spine inferiorly. Bony landmarks of the skull, scapulae, clavicles, and upper ribs define the attachment field. Clean geometry. This posterior perspective matters when teaching scapulothoracic mechanics because you can track how fiber direction predicts function: descending fibers elevate and upwardly rotate the scapula, transverse fibers retract it, and ascending fibers depress and assist upward rotation. Clinically, it supports discussion of spinal accessory nerve (CN XI) injury, where trapezius denervation leads to shoulder droop, lateral scapular winging with impaired overhead abduction, and loss of effective scapular upward rotation. The view also aligns with common myofascial pain patterns in the upper trapezius at the cervicothoracic junction and along the medial border of the scapular spine.
