Description
Spanning the posterior neck and upper thorax, the trapezius is presented as a superficial sheet beneath the skin, with its descending (upper), transverse (middle), and ascending (lower) fibers distinguished by their oblique, horizontal, and superolateral directions. Superior fibers arise from the external occipital protuberance and nuchal ligament, then course inferolaterally toward the lateral third of the clavicle; the middle fibers run medially to laterally toward the acromion and spine of the scapula. Inferiorly, ascending fibers originate from the spinous processes of the lower thoracic vertebrae and pass superolaterally to the medial end of the scapular spine, framing the posterior contour of the shoulder girdle around the scapulae and upper ribs. Posterior trapezius anatomy matters because scapular position is often the limiting factor in shoulder mechanics, not the glenohumeral joint alone. Weakness or denervation of the upper trapezius from spinal accessory nerve (CN XI) injury, classically after posterior triangle neck dissection or lymph node biopsy, produces shoulder droop and impaired upward rotation during arm elevation, with trapezius wasting visible along the neck to acromion line. This posterior view also supports teaching of myofascial pain patterns, where trigger points in the descending trapezius commonly refer pain to the temporal region and lateral neck.
