Description
Oriented from a posterior perspective, the paired kidneys sit high in the retroperitoneum with the suprarenal (adrenal) glands capping their superomedial poles. Medial to each renal hilum, the abdominal aorta lies left of midline and the inferior vena cava lies to the right, giving rise to the renal arteries and receiving the renal veins as they course laterally toward the kidneys. Each ureter descends inferiorly along the posterior abdominal wall, and a ureteric nerve (via the renal and aortic plexuses, continuing into the superior hypogastric plexus) tracks longitudinally with it toward the pelvic brim and urinary bladder. Lymphatic channels and nodes accompany the great vessels and the renal pedicles. Posterior views like this earn their keep when you need to teach or plan around retroperitoneal relationships: the ureter’s proximity to the gonadal vessels, iliac bifurcation, and autonomic plexuses explains both patterns of referred pain and common iatrogenic injury sites. Sensory afferents traveling with sympathetics (T11 to L2) underpin the classic flank-to-groin colic of ureteral calculi, while peri-ureteric dissection during colorectal surgery, aortoiliac exposure, or lymphadenectomy can disrupt the delicate autonomic input that modulates ureteral peristalsis.
