Description
Centered on the retroperitoneal urinary tract, the paired kidneys sit high and lateral with the suprarenal glands capping their superior poles, while the abdominal aorta lies slightly left of midline and the inferior vena cava runs to its right. Each ureter descends inferiorly from the renal pelvis, crossing anterior to the psoas region toward the pelvic brim and then continuing to the urinary bladder in the midline. Fine ureteric nerves are traced along the ureter’s adventitia as delicate strands, closely applied to the ureteral wall rather than traveling as a separate cord. Renal arteries branch laterally from the aorta and renal veins drain medially to the vena cava. Small structures matter here. Autonomic innervation of the ureter, via contributions from the renal, aortic, and hypogastric plexuses, is clinically tied to ureteral peristalsis and the pain patterns of ureterolithiasis that can refer from the flank toward the groin as the calculus migrates distally. The anterior perspective also supports operative planning: during ureterolysis, pelvic lymph node dissection, or anterior approaches to the distal ureter, traction or thermal spread can strip the adventitia and disrupt the periureteral nerve plexus along with segmental blood supply. That combination raises the risk of ischemic stricture or postoperative dysmotility. Urology and general surgery teaching modules on retroperitoneal anatomy can use this artwork to orient learners to the ureter’s course relative to the great vessels and to explain why dissection should stay close to the ureteral sheath. It also fits well in atlas plates and journal figures addressing ureteric colic, iatrogenic ureteral injury, or pelvic surgery nerve-sparing principles in the male pelvis.
