Description
Centered in an anterior view of an obese adult male in anatomical position, the paired kidneys sit retroperitoneally on either side of the vertebral column, with the right kidney slightly inferior to the left as expected from hepatic mass. Each renal pelvis funnels into a ureter that descends inferiorly along the posterior abdominal wall, then courses into the pelvis toward the urinary bladder. The bladder occupies the midline of the lesser pelvis, immediately posterior to the pubic symphysis and superior to the proximal urethra, while abdominal adiposity increases the apparent depth between the ventral body wall and retroperitoneal organs. Body habitus changes how the urinary tract is taught, examined, and accessed. Increased visceral and subcutaneous fat can obscure surface landmarks for suprapubic bladder palpation and alter sonographic windows, and it adds working distance and limited exposure during ureteral reimplantation, cystoscopy positioning, or laparoscopic/robotic nephrectomy. Hydroureteronephrosis from a distal ureteral calculus is also harder to correlate clinically when pain localization and exam findings are blunted by obesity. Orientation matters.
