Description
Posteriorly, the knee joint is presented with the distal femur superior to the proximal tibia, and the fibular head positioned laterally. The arcuate popliteal ligament is highlighted as a fibrous thickening of the posterior capsule with an inverted Y configuration, arising near the fibular head and sweeping superomedially and superolaterally toward the posterior aspect of the lateral femoral condyle and adjacent capsule. Deep to this capsuloligamentous layer, the popliteus tendon courses obliquely from the lateral femoral condyle toward its insertion on the posterior tibia. Clear orientation. Arcuate complex anatomy matters because it is one of the structures that resists posterolateral rotatory instability, working alongside the popliteus tendon, popliteofibular ligament, and lateral (fibular) collateral ligament to control external rotation and varus at the tibiofemoral joint. Injury patterns often follow a varus hyperextension mechanism and can be missed if attention stays on the cruciate ligaments alone, leading to persistent instability after isolated ACL reconstruction. This posterior view supports surgical planning by clarifying where the arcuate ligament blends with the posterolateral capsule near the fibular head, a region where dissection must respect the nearby course of the common fibular (peroneal) nerve around the fibular neck.
