Description
Posterior to the knee joint, the distal femur expands into the medial and lateral femoral condyles, separated by the intercondylar fossa and resting superior to the tibial plateau of the proximal tibia. Inferior and slightly lateral, the head of the fibula marks the posterolateral corner, while the patella sits anterior to the femoral trochlea and is largely hidden from this perspective. Draped across this osseous framework, the popliteal artery runs deep and centrally within the popliteal fossa, accompanied by the popliteal vein, with the femoral vessels continuing proximally and the great saphenous vein tracking more superficially along the medial side. Clear spatial layering. Surgical planning around the posterior knee depends on understanding these relationships because the popliteal artery lies directly against the posterior capsule and femur, with limited soft-tissue protection during posterior approaches or arthroscopic instrumentation. Popliteal artery injury is a feared complication in total knee arthroplasty and high-energy knee dislocations, and this view helps clarify why the vessel is tethered at the adductor hiatus proximally and the soleal arch distally. The genicular anastomosis around the condyles and tibial plateau also explains collateral flow when the superficial femoral artery is occluded and why pulses can be misleading in some trauma presentations.
