Description

Posterior anatomy of the male larynx continues inferiorly as the trachea, with its C-shaped hyaline cartilage rings and posterior membranous wall, before dividing at the carina into the right and left main bronchi. From this dorsal perspective, the right main bronchus runs more vertical and slightly wider than the left, then gives rise to lobar (secondary) bronchi and segmental (tertiary) bronchi that spread laterally into the lung fields as the bronchial tree. Proximally, the airway column stays near the midline; distally, branching becomes increasingly peripheral as bronchi transition toward bronchioles. A posterior view is the cleanest way to teach the airway’s branching logic without the visual obstruction of the sternum and anterior mediastinal structures, and it aligns well with how the trachea and main bronchi are approached in bronchoscopy. The asymmetric geometry matters clinically: aspirated material and misplaced endotracheal tubes enter the right main bronchus more often, and the carina remains a key landmark for tracheobronchial injury, subcarinal lymphadenopathy, and staging of bronchogenic carcinoma.
