Description
Prominent in this posterior view is the calcaneal tuberosity (tuber calcanei), with its broad, roughened superior region forming the insertion footprint for the calcaneal (Achilles) tendon. The posterior surface rises superiorly toward the posterior articular facet for the talus, while the plantar aspect expands into the medial and lateral processes that contribute to the weight-bearing heel. Laterally, the peroneal (fibularis) trochlea may be appreciated as a bony ridge that separates the fibularis longus and brevis tendon grooves, and inferiorly the plantar surface transitions to the area continuous with the plantar fascia attachment. Big bone. Clear landmarks. For clinicians, the posterior calcaneus is where tendinopathy becomes anatomy you can palpate. The relationship between the Achilles insertion and the posterosuperior calcaneal prominence is central to Haglund deformity with retrocalcaneal bursitis, and it also explains why insertional Achilles tendinopathy often presents with pain and swelling just superior to the tuberosity. Orthopaedic approaches for calcaneal fractures and hindfoot reconstruction also hinge on these contours, since the tuber is a traction point for the gastrocnemius-soleus complex and displacement alters hindfoot height and alignment.
