Description
Anteriorly, the calcaneus presents its broad anterior articular surface for the cuboid, forming the calcaneocuboid component of the transverse tarsal (Chopart) joint. Medial to this, the sustentaculum tali projects superomedially, supporting the middle talar facet and creating a groove inferiorly for the tendon of flexor hallucis longus. The lateral wall sits more laterally and slightly posterior, while the bulky calcaneal body expands inferiorly toward the plantar surface. Subtle cortical contours hint at the plantar and posterior tuber where the calcaneus thickens proximally. An anterior perspective helps clarify how hindfoot load transfers into the lateral column through the calcaneocuboid joint, a relationship that gets distorted in intra-articular calcaneal fractures with anterior process involvement and lateral wall blowout. It also frames the sustentaculum tali as a fixed medial fragment in many fracture patterns, a key concept when planning reduction and screw trajectories. A common pain generator, the calcaneocuboid joint can be involved in midfoot sprains and post-traumatic arthrosis, and this view makes joint congruity straightforward to explain. Clear landmarks.
