Description
Medial aspect of the hindfoot is centered on the calcaneus, with the fracture line crossing the body of the heel bone near the sustentaculum tali (talar shelf). Superior to the calcaneus sits the talus, whose posterior and middle subtalar articular facets oppose the calcaneal facets; thin blue cartilage bands outline these joint surfaces. Anteriorly, the calcaneus meets the cuboid at the calcaneocuboid joint, while the navicular and metatarsals extend distally to complete the medial-to-lateral arch profile. Red emphasis localizes the bony disruption and adjacent reactive change. Calcaneal fractures often propagate into the posterior subtalar joint, and this medial perspective makes the relationship to the sustentaculum tali easier to teach than a lateral view, because the sustentaculum frequently remains relatively stable while the lateral wall and posterior facet displace. Alignment here matters. Loss of subtalar congruity predicts post-traumatic subtalar arthritis, persistent hindfoot valgus, and altered Achilles tendon moment arm, and it also influences operative planning when percutaneous screw trajectories are aimed toward the sustentaculum to restore height and joint surface. Medial hindfoot anatomy also frames common complications, including irritation of the flexor hallucis longus tendon as it grooves the posterior calcaneus and neuritic symptoms if swelling tracks toward the tarsal tunnel.
