Description
Medial aspect of the human calcaneus is presented with the sustentaculum tali projecting medially from the superior calcaneal body, creating a shelf beneath the talus. Posteriorly, the calcaneal tuberosity occupies the heel and continues anteriorly into the plantar surface, while the anterior articular region approaches the calcaneocuboid joint. Superior facets for the talus sit above, and the plantar groove under the sustentaculum aligns with the course of the flexor hallucis longus tendon. Bony landmarks dominate. This medial perspective matters because the sustentaculum tali is a primary landmark in hindfoot biomechanics and in trauma patterns such as intra-articular calcaneal fractures that disrupt the posterior subtalar joint. Fracture lines, malunion, or subtalar arthritis alter the relationship between the sustentaculum and the talar facets, and surgeons often reference the sustentaculum as a relatively constant fragment during open reduction and internal fixation. The same region becomes clinically concrete when evaluating tarsal tunnel syndrome, where swelling or post-traumatic deformity near the medial calcaneus can contribute to tibial nerve compression beneath the flexor retinaculum.
