Viewed from the inferior aspect, the calcaneus dominates the hindfoot, with the calcaneal tuberosity positioned posteriorly and the anterior process projecting toward the midfoot to meet the cuboid at the calcaneocuboid joint. Medially, the sustentaculum tali forms a shelf for the talus and creates the floor of the tarsal tunnel region, while laterally the peroneal (fibular) trochlea marks the course of the fibularis longus and brevis tendons. The plantar surface shows the characteristic roughened areas for the plantar fascia and intrinsic plantar musculature, contrasted against smoother articular facets for the talus superiorly and the cuboid anteriorly. Orientation cues typically place the heel posterior and the forefoot anterior, with medial and lateral borders clearly differentiated by the sustentaculum tali and peroneal trochlea.
