Description
Centered on the lateral column of the foot, the cuboid is presented from the inferior (plantar) aspect with its plantar surface facing the viewer and the dorsal surface oriented superiorly. A prominent oblique sulcus for the fibularis (peroneus) longus tendon crosses the plantar cuboid from posterolateral toward anteromedial, bounded by plantar ridges that form a pulley-like channel. Posteriorly the cuboid abuts the anterior calcaneus at the calcaneocuboid joint, while its distal facets face the bases of the fourth and fifth metatarsals and its medial surface relates to the lateral cuneiform and navicular region. Lateral and medial plantar landmarks are kept crisp. That plantar groove matters. Fibularis longus runs through it after passing inferior to the peroneal trochlea of the calcaneus, then turns medially toward the base of the first metatarsal and medial cuneiform, so the cuboid acts as a bony fulcrum for plantarflexion of the first ray and support of the transverse arch. Pain and tenderness over the plantar cuboid often enters the differential for cuboid syndrome, peroneal tendinopathy, and injuries of the calcaneocuboid ligament complex after an inversion sprain. The inferior perspective also clarifies the orientation of the calcaneocuboid joint line relevant to lateral column lengthening procedures and midfoot arthrodesis planning. Podiatry and orthopedic teaching modules on midfoot biomechanics will find this angle useful when explaining the peroneus longus pulley mechanism and lateral column articulations.
