From an inferior perspective, their wedge geometry is apparent, with the broader plantar aspects contributing to the transverse arch while their smaller dorsal surfaces would lie superiorly. Medially the medial cuneiform aligns with the first ray, while the intermediate and lateral cuneiforms lie progressively more lateral, approaching the cuboid at the lateral midfoot. Articular margins are defined by hyaline cartilage at the intercuneiform and cuneometatarsal joints. This angle matters because it clarifies how the cuneiforms lock the midfoot during toe-off and how small shifts at the Lisfranc complex can destabilize the medial column. Subtle diastasis between the medial cuneiform and the base of the second metatarsal, or between the cuneiforms themselves, is a common pattern in Lisfranc injury and can be missed without a solid mental model of the plantar architecture.
