Description
Seen from the lateral aspect of an adult male foot, the intermediate cuneiform (os cuneiforme intermedium) sits in the medial midfoot between the navicular proximally and the base of the second metatarsal distally, its wedge profile tapering plantarward. The calcaneus and talus form the hindfoot posteriorly, while the navicular lies anterior to the talar head and leads into the cuneiform row; the cuboid and lateral metatarsals fall more lateral and plantar in the same bony chain. Blue articular cartilage marks the joint surfaces, including the cuneonavicular interface and the tarsometatarsal surfaces, clarifying how the intermediate cuneiform aligns with the second ray. This lateral perspective matters because the intermediate cuneiform is a keystone for the Lisfranc (tarsometatarsal) complex, where subtle malalignment between the cuneiforms and the second metatarsal base can signal an unstable injury.
