Description
Plantar (inferior) orientation of the male foot skeleton emphasizes the five metatarsals, highlighted in blue from their proximal bases to their distal heads, aligned between the tarsal row proximally and the phalanges distally. Medially, the first metatarsal sits shorter and more robust, while the second metatarsal tracks more proximally into the Lisfranc region between the cuneiforms, and the fifth metatarsal flares laterally toward its tuberosity. Across the forefoot, the metatarsal heads form the transverse arch, with the intermetatarsal spaces separating adjacent shafts and leading into the metatarsophalangeal joints. Plantar bony contours are where forefoot mechanics become tangible. The inferior surfaces of the metatarsal heads relate directly to plantar plate and flexor tendon loading, and they are the hard substrate beneath common pain generators like metatarsalgia, plantar plate tears at the second metatarsophalangeal joint, and sesamoiditis under the first ray. This view also supports teaching of load transfer through the first metatarsal during push-off and why a long second metatarsal can predispose to Morton neuroma in the third intermetatarsal space. Clear landmarks.
