Description
Seen in lateral profile, the pedal skeleton is oriented with the calcaneus posterior and plantar, the talus stacked superiorly to form the ankle mortise, and the tarsals stepping forward into the midfoot. The five metatarsals are emphasized, their bases articulating proximally with the cuneiforms and cuboid at the tarsometatarsal joints, and their heads aligning distally at the metatarsophalangeal joints with the proximal phalanges. From this side view, the shafts and relative lengths of the metatarsals read clearly as they angle anteroinferiorly toward the toes, with the phalanges extending distally in longitudinal rays. Color coding isolates the metatarsals against the hindfoot and midfoot bones. Lateral alignment matters because it is the simplest way to teach the medial longitudinal arch and the load path from the talus to the forefoot, and it frames how the metatarsal heads meet the ground during gait. Stress fractures of the second and third metatarsal shafts, common in runners and military recruits, can be understood biomechanically when you appreciate how those central rays sit between the more mobile first and fifth rays.
