Capsule of the Styloid, Posterior View

Description

Positioned in a posterior (dorsal) view of the male wrist and hand, the carpal skeleton is seen with the distal radius and ulna proximal to the proximal carpal row, and the metacarpals extending distally toward the phalanges. Along the ulnar side, the styloid process of the ulna projects distally as a palpable bony landmark, with a capsular sleeve and synovium implied around the adjacent distal radioulnar region and ulnocarpal articulation. Dorsal joint lines between the carpal bones and the metacarpophalangeal and interphalangeal joints are defined, with the phalanges aligned distal to the metacarpals in a relaxed, partially open posture. Bony surfaces and apophyses are rendered with cortical texture and subtle porosity. Posterior access to the ulnar styloid and its capsular attachments matters when teaching stability at the distal radioulnar joint and ulnar-sided wrist pain patterns. The ulnar styloid anchors components of the triangular fibrocartilage complex (TFCC) and the dorsal and volar radioulnar ligaments, so fractures at the styloid base or tears near the foveal insertion can present with DRUJ instability and pain during pronation-supination. A clear dorsal relationship between the ulnar styloid, carpus, and metacarpals also supports procedural orientation for arthroscopy portals and for explaining why ECU subsheath pathology and TFCC lesions often coexist.
