Description
Positioned on the lateral aspect of the distal radius, the radial styloid process projects distally beyond the ulnar head and forms the lateral bony contour of the posterior wrist. From this posterior perspective, the carpal rows sit distal to the radius, with the scaphoid and lunate articulating at the radiocarpal joint and the triquetrum and pisiform more ulnar. Metacarpals I to V extend distally into the digits, each continuing into proximal, middle, and distal phalanges (two phalanges in the thumb). Joint lines at the radiocarpal, midcarpal, carpometacarpal, metacarpophalangeal, and interphalangeal levels are cleanly defined. A sharp landmark. Clinically, the radial styloid matters because it anchors the radial collateral ligament of the wrist and the brachioradialis tendon, and it sits immediately adjacent to the first dorsal extensor compartment containing abductor pollicis longus and extensor pollicis brevis. Pain and tenderness at or just proximal to the styloid often point toward de Quervain tenosynovitis, while a prominent or displaced styloid can alter carpal alignment after distal radius fractures, including Barton and Colles patterns, with downstream effects on radioscaphoid loading and scapholunate mechanics.
