Description
Running between adjacent ribs, the external intercostal muscles occupy the intercostal spaces along the posterior thoracic wall, lying superficial to the internal intercostals and deep to the scapular and paraspinal musculature that would normally overlie them. From this posterior perspective, the thoracic vertebrae sit in the midline, with ribs curving laterally from their heads and tubercles at the costovertebral and costotransverse joints toward their angles. Muscle fibers are oriented infero-anteriorly from the inferior border of each rib to the superior border of the rib below, a direction that is easiest to appreciate close to the rib angles. Bony landmarks dominate. External intercostals contribute to rib elevation during inspiration, and their posterior extent helps explain why dorsal thoracic trauma can disrupt ventilatory mechanics even when the anterior chest wall appears intact. Clinically, this posterior rib and interspace anatomy frames needle and tube thoracostomy planning by reinforcing where the neurovascular bundle runs, along the inferior margin of each rib in the costal groove, and why approaches aim just superior to a rib to reduce intercostal artery and nerve injury. It also clarifies the relationship of the rib head and neck to the sympathetic chain and dorsal rami near the vertebral column. A practical view.
