Seen from a posterior thoracic perspective, the internal intercostal muscles occupy the intercostal spaces between adjacent ribs (costae), lying deep to the external intercostals and spanning from the inferior border of the rib above to the superior border of the rib below. Their fibers course inferoposteriorly, and the muscular portion is densest in the posterior intercostal spaces near the vertebral column, where the ribs articulate at the costovertebral and costotransverse joints. Medially, the rib angles and thoracic spinous processes serve as clear landmarks, with the scapulae and posterior ribs framing the highlighted musculature laterally. This posterior emphasis matters because the internal intercostals contribute to forced expiration by depressing the ribs, and their relationship to the neurovascular bundle (intercostal vein, artery, and nerve) along the inferior margin of each rib is easiest to teach when the intercostal spaces are cleanly separated. The transition into the internal intercostal membrane toward the vertebral column also anchors a common procedural teaching point: posterior approaches to thoracentesis, chest tube placement, and intercostal nerve blocks must respect rib level and needle position to avoid the neurovascular bundle.
