Description
Seen from a lateral thoracic perspective, the internal intercostal muscles appear as segmental sheets spanning adjacent costae within the intercostal spaces, occupying the anterolateral rib cage from the parasternal region toward the rib angles. Their fibers run obliquely inferoposteriorly (down and back) from the inferior border of a rib to the superior border of the rib below, contrasting with the external intercostal fiber direction. Superficial bony landmarks include the sternum anteriorly, the costal cartilages at the anterior rib ends, and the vertebral column posteriorly, with the rib angles marking the typical transition where internal intercostals become less muscular. Orientation matters here because internal intercostals are the primary intercostal layer generating forced expiration, drawing the ribs inferiorly and reducing thoracic volume, while the interchondral (parasternal) portion can assist inspiration. That distinction becomes clinically concrete in acute asthma or COPD exacerbations, when recruitment of intercostal musculature and paradoxical indrawing of interspaces can be appreciated on exam. This view also aligns with procedural anatomy: intercostal neurovascular bundles course along the inferior margin of each rib in the costal groove between the internal and innermost intercostals, a key relationship when placing a chest tube or performing an intercostal nerve block. Small margin for error.
