Description
Seen from the lateral thoracic wall, the innermost intercostals appear as a thin, discontinuous muscle layer spanning the intercostal spaces on the deep aspect of the rib cage. Muscle fibers run obliquely between adjacent costae, with segmental slips separated by intervals where only internal intercostal membrane and endothoracic fascia intervene. Posteriorly, the slips approach the angles of the ribs near the partially visualized thoracic vertebral column, while inferiorly the field drops toward the upper pelvis, reinforcing overall body orientation. Recognition of the innermost intercostals matters because they sit immediately superficial to the endothoracic fascia and parietal pleura, a relationship that frames procedural risk. During tube thoracostomy and intercostal nerve blocks, the neurovascular bundle (intercostal vein, artery, nerve) courses in the costal groove along the inferior border of each rib, between the internal intercostal and innermost intercostal layers; this is why needle and trocar placement is typically kept just superior to the upper edge of the rib below. Their discontinuity also helps explain why the “internal intercostal” plane is not uniform along the thoracic wall, and why the transversus thoracis (anteriorly) and subcostales (posteriorly) can be discussed as related deep-layer variants.
