Description
Presented on the left hemithorax in lateral profile, the innermost intercostal muscles occupy the deeper layer of the intercostal spaces between adjacent costae, immediately internal to the internal intercostals and external to the parietal pleura. Muscle slips run obliquely from the inferior border of the rib above to the superior border of the rib below, paralleling the fiber direction of the internal intercostals. Their bulk is concentrated in the intermediate and lateral parts of each intercostal space, while the region near the vertebral column is dominated by other deep thoracic wall muscles. Rib contours and the thoracic vertebrae provide the bony frame for orientation. Functional anatomy matters here because the innermost intercostals are easiest to misunderstand as a uniform, continuous layer when they are segmental and interdigitate with adjacent planes of the thoracic wall. That distinction becomes practical during pleural procedures, when a needle or tube traverses skin, superficial fascia, serratus anterior and/or latissimus dorsi (depending on level), then the intercostal muscle layers before approaching pleura. Keep the intercostal neurovascular bundle in mind: the main VAN bundle typically lies in the costal groove along the inferior border of the superior rib, while collateral branches may course near the superior border of the inferior rib. Small margins matter. Use this illustration in gross anatomy and respiratory mechanics teaching to clarify deep intercostal layering, fiber direction, and the lateral concentration of the innermost intercostals in a male thorax. It also fits procedural training materials for thoracostomy, thoracentesis, and intercostal nerve block, where safe trajectories depend on knowing what sits deep to each rib and where the bundle runs.
