Description
Running between adjacent costae as the deepest layer of the intercostal spaces, the innermost intercostal muscles are shown deep to the internal intercostals and superficial to the endothoracic fascia and parietal pleura. Fibers course obliquely inferoposteriorly from the inferior border of each rib toward the superior border of the rib below, most evident laterally where the layer is best developed. Posteriorly, the bundles lie near the costal angles and thoracic vertebrae, while anteriorly they approach the sternum but give way to intercostal membranes. Deep. Bony landmarks anchor the anatomy: ribs articulating with the thoracic vertebrae, the sternum at midline, and the thoracolumbar junction leading into the pelvis. That context matters because the innermost intercostals share a neurovascular plane with the internal intercostals, and the intercostal vein, artery, and nerve travel in the costal groove along the inferior margin of each rib, classically between internal and innermost layers. That is the danger zone during tube thoracostomy and thoracentesis, where you aim just superior to a rib to avoid injuring the intercostal neurovascular bundle and causing hemorrhage or neuralgia.
