Description
Running within each intercostal space, the external intercostal muscles span from the inferior border of a rib above to the superior border of the rib below, their fibers coursing infero-anteriorly from the vertebral column toward the anterior thoracic wall. Posteriorly, the muscle bellies appear thickest near the costovertebral region, closely related to the thoracic spinous processes and costal angles. Approaching the sternum, the contractile fibers taper and continue as the external intercostal membrane, leaving the anteromedial interspaces dominated by deeper layers. Ribs, thoracic vertebrae with intervertebral discs, and the superior pelvis and sacrum provide bony landmarks and orientation. External intercostals are a teaching workhorse for mechanics of ventilation because their fiber direction explains how rib elevation increases thoracic volume during inspiration.
