Posteriorly, the fibular (peroneal) vein is traced along the lateral compartment of the leg, coursing adjacent to the posterior aspect of the fibula and deep to the superficial calf musculature. Proximally it parallels the fibular artery and tracks with the deep fibular (peroneal) neurovascular bundle, while distally it receives venous tributaries from the tarsal region before continuing superiorly toward the tibioperoneal trunk. Surrounding landmarks logically include the gastrocnemius and soleus posteriorly, with fibularis longus and brevis positioned more laterally and anteriorly, and the tibia situated medial to the fibular shaft. Small venules and communicating channels are shown converging into the paired deep vein pathway. Deep and superficial planes are easy to separate here. A posterior perspective on the peroneal vein matters because it is a common site of isolated distal deep vein thrombosis and can be overlooked on limited compression ultrasound that focuses on the popliteal and femoral segments.
