Description
Oriented from a posterior perspective, the fibular (peroneal) veins run deep in the posterior compartment of the leg, closely accompanying the fibular artery along the medial aspect of the fibula and posterior to the lateral malleolar region as they descend toward the ankle. The tibia lies more medial, the fibula more lateral, with the knee region superiorly framed by the distal femur, patellae, and proximal tibiofibular relationships. Superficial venous pathways, including the great saphenous vein coursing more anteromedially, contrast with the deep peroneal venous channels that remain tethered to the interosseous membrane and adjacent musculature. Venous valves are suggested along the deep vessel course. Deep and superficial systems are easy to separate here. Posterior leg venous anatomy matters when you are tracing sources of distal deep vein thrombosis and planning venous duplex protocols that can miss calf thrombi if the peroneal veins are not interrogated. Their tight pairing with the fibular artery and location adjacent to the fibula explain why fractures, compartment syndrome, or operative approaches to the lateral leg can threaten both arterial inflow and venous outflow, with secondary swelling and pain that may mimic isolated muscular injury. The view also supports teaching of how peroneal veins contribute to the tibioperoneal trunk and ultimately the popliteal vein, clarifying why symptoms can propagate proximally.
