Description
Running deep in the lateral compartment of the leg, the fibular (peroneal) vein is traced alongside the fibula and posterior to the lateral malleolus region, then courses proximally toward the popliteal fossa. Medially, the great saphenous vein is visible in the superficial fascia, ascending anterior to the medial malleolus and along the medial tibial border toward the femoral triangle. Smaller superficial tributaries and perforating veins bridge superficial to deep pathways, and venous valves are indicated along the long ascending channels. Appreciating the fibular vein in an anterior projection helps clarify a common source of confusion for learners, the peroneal vessels sit laterally yet remain deep, often paired as venae comitantes around the fibular artery rather than tracking with the superficial saphenous systems. That relationship matters in deep vein thrombosis of the calf, where thrombus may involve the paired deep veins (fibular and posterior tibial) while the great saphenous remains patent, and it also frames the anatomy for duplex ultrasound mapping and interpretation.
