Description
Running deep along the posterolateral compartment of the right leg, the fibular (peroneal) vein is shown ascending adjacent to the fibula, typically as paired venae comitantes accompanying the fibular artery. Distally, it courses superior to the lateral malleolus region and remains deep to the fibularis (peroneus) longus and brevis, while more superficially the small saphenous vein tracks along the posterior midline of the calf toward the popliteal fossa. Proximally, the fibular vein converges with the anterior and posterior tibial veins to form the popliteal vein. Deep and superficial systems are contrasted clearly. Lateral leg venous anatomy matters when you are tracing the source and propagation pathway of deep vein thrombosis in the calf, where thrombus can originate in the deep crural veins and extend into the popliteal vein. The fibular vein is also a practical landmark during posterior or posterolateral approaches to the fibula and ankle, where retraction or injury to the peroneal vessels can complicate open reduction and internal fixation. Calf perforators and the relationship between superficial and deep channels also underpin venous reflux mapping and procedure planning. Small vessel, big consequences.
